Recorded IV Contrast in MRI: Gadolinium: What Are You Really Doing?

This lecture is directed towards all but especially newer technologists who did not experience the “NSF” scare of 10+ years ago when the condition became a very real concern for those of us who were responsible for administering the agent. Newer technologists have more than likely heard of the condition (NSF) but have not experienced it firsthand. Giving some real meaning to the condition is part of the objectives of this talk. The cause of the condition known as NSF (Nephrogenic Systemic Fibrosis) from gadolinium separating from its chelating agent is/will be explained. The name of the condition, NSF, will be broken down into its individual components.

This discussion will explain and introduce to the technologists, what Gadolinium is and how Gadolinium and some other contrast agents are used in the radiology department work. Also covered is why, many times, during the training phase of a technologist, the way/how/why the different contrasts work is skimmed over. Administering I.V. contrast, its removal from the body, and what happens to gadolinium once it is removed from the patients’ bloodstream and where it goes will also be discussed.

  1. Define and explain the use and mechanisms of intravenous (IV) contrast agents in imaging studies.
  2. Identify and differentiate between ligands, chelating agents, and various types of IV contrast used in MRI.
  3. Describe the key characteristics and implications of Nephrogenic Systemic Fibrosis.
  4. Analyze the factors that influence a patient's estimated Glomerular Filtration Rate (eGFR).
  5. Evaluate the impact of gadolinium utilization on the environment and its potential consequences.
