Type 2 Diabetes and Obesity, Fifth Edition
Obesity and diabetes are both increasing in the U.S. population. Obesity is a strong predictor of type 2 diabetes (T2D), although everyone who is obese does not have diabetes.
Medical nutrition therapy has been considered foundational for people with or at risk for T2D and has been shown to have a significant impact on weight management, glycemia, hypertension, and lipid profiles in these populations. The complexity of nutrition and the often-mixed messages on what is optimal nutrition among people with or at risk for diabetes has led to confusion for individuals with these conditions and their health care teams. Effective nutrition therapy should be individualized to the person by a registered dietitian nutritionist (RDN) trained and experienced in all aspects of diabetes care and obesity management
The goal of this CPE program is to provide the nutrition professional with an understanding of the pathology of these conditions. In addition, the nutrition professional will develop competency in designing strategies to prevent or treat obesity and T2D.
Chapter 1 describes the normal physiology of tissues and systems involved in the person without obesity or T2D. In Chapter 2, the pathophysiology of obesity and diabetes is explained. The classification, diagnosis, and recommended screening practices for T2D and obesity are included in Chapter 3.
Medical Nutrition Therapy for T2D and its application to obesity are the focus of Chapter 4, including information on individualizing the macronutrient distribution and encouraging healthy fat, protein, and carbohydrate choices Information about whole grains, fiber, nonnutritive sweeteners, and the glycemic index are also included. Chapter 5 provides an overview of meal planning tools, weight loss and diabetes programs, counseling strategies, and recommendations on monitoring progress.
Exercise and physical activity are important considerations in both T2D and obesity. The role of aerobic and strength training in these conditions is explored in Chapter 6. Equally important is the knowledge of medications used to treat diabetes and obesity, which are included in Chapter 7. Chapter 8 provides information about metabolic/bariatric surgery, which has become an important treatment for both obesity and T2D. Chapter 9 provides a concise overview of frequent comorbidities and how these may change medical nutrition therapy and physical activity recommendations.
Clinical implications are highlighted in each chapter. Case studies integrate the information from these chapters and provide examples of diagnosis and treatment. Also included is a listing of additional sources for your continued learning as well as organizations that are resources for additional information or updates.

- Integrate knowledge of possible comorbidities into the counseling of those with T2D and/or obesity.

Janice MacLeod is a diabetes-cardiometabolic consultant and key opinion leader in digital health and chronic condition management. Janice began her career as a diabetes dietitian and certified diabetes care and education specialist at Carilion Health System in Roanoke, VA and later at the University of Maryland in Baltimore, MD. Currently Janice is partnering with the Apostle Group, a CMS Care Transformation organization, developing an evidence strategy for their Food as Medicine research. Janice previously led clinical advocacy at Medtronic/Companion Medical and has served in clinical leadership roles with Welldoc, a pioneer digital health company and Johnson and Johnson, including as senior diabetes medical science liaison. She is a recent past chair of the Academy of Nutrition & Dietetics (Academy) Diabetes Practice Group and has been elected to the Association of Diabetes Care and Education Specialists Board of Directors, 2024-2027. She has served on the American Diabetes Association (ADA) Scientific Sessions planning committees and was a co-author of the ADA Diabetes Nutrition Consensus Report as well as the Academy Nutrition Practice Guideline for Type 1 and Type 2 Diabetes. Janice has authored multiple publications and developed numerous presentations and continuing education programs on diabetes nutrition, digital health, and practice transformation. Her passion is developing business-savvy solutions leveraging technology to transform care.
